Health Care Provider Hub


As a health care provider in Delaware, you’ll play an important role in the new Delaware Paid Leave claims process. When a Delaware employee files for medical or family caregiver leave, their health care provider will be required to complete a Certification of Serious Health Condition (CSHC) before the employer can adjudicate the claim.

You may already be used to completing certifications for the Family and Medical Leave Insurance Program (FMLA). The CSHC is similar, but it’s accessed and completed online.

When an employee files for medical or family caregiver leave, it kicks off this process:

  1. Delaware LaborFirst checks for ineligible claims.
  2. The Employee receives the CSHC access form and code.
  3. Employee provides the CSHC access form and code to you, their health care provider.
  4. You or your staff will use the code to access Delaware LaborFirst and complete the CSHC.
